HOW DO I MAKE AN APPOINTMENT?
First, please go to CONTACT ADINA and fill out the short form. This will give me an idea of what's going on and a way to contact you. Next, let's chat. It's important that it feels like the right fit for you. I offer a free phone consultation (upon request). If you would like to proceed, we'll set an appointment. Once we've set the appointment time, please download and complete the INTAKE FORM below prior to our first session. Please take a moment to review the forms at the bottom of this page to review HIPAA/Confidentiality Statements and network specific forms if using EAP. If you have difficulties accessing/printing the form, please complete the BRIEF INFO FORM and you will complete the remaining paperwork at the first session.
HOW DO I PAY FOR SERVICES?
Private Pay: My rate is $140 per session. While I do not offer a sliding fee scale, I offer 2 slots per week at a reduced rate. Please inquire about availability. If you have insurance, I can provide you an invoice/superbill to submit to your insurance company for out of network reimbursement. This is done by request only.
EAP/Insurance: If using insurance and I am in network with your carrier or if you are participating in your company's Employee Assistance Program (EAP), I am obligated to my contracted rate and you are obligated to pay your deductible or co-payment at each session, if applicable. You are responsible for determining insurance eligibility prior to our first session. This can be done by calling the phone number on the back of the card for behavioral health services.
Cash, credit card or personal checks are acceptable methods of payment and I will provide a receipt for all fees paid upon request. A fee of $35.00 will be charged for returned checks.
Life happens. I understand that. Please give as much notice as possible for cancellations as I normally have people on a waiting list. A 24 hour notice of cancellation is preferred. 2 or more “no-shows” for your appointment will result in a missed appointment fee of $100.00. If you are late for a session, you will still be charged the customary session rate. More than 2 late cancellations (within 6 hours or your appointment time) will be treated as a no show and charged accordingly. If you will be more than 20 min late, please reschedule.
WHAT WILL I HAVE TO DO?
Developing a productive therapeutic relationship is paramount to positive treatment outcomes. Therefore, it is important to consider the credentials, therapeutic orientation, and personality of your therapist, taking into account what you (the individual, couple and/or family) desire from your provider of therapeutic services. Therapy is designed to offer insight, awareness, motivation for change, empowerment, alternate perspectives and possibilities for addressing the identified problem, thereby fostering healthy relationship with self and others. The majority of work is done outside of the office and requires active preparation and participation on the part of you/your family. Therapy is very challenging and during the course of treatment, sensitive matters will be discussed. This may initially intensify the emotions, thoughts and/or behaviors experienced but this is normal and typically required for desired change. It is a normal part of the therapeutic process to feel scared, angry and vulnerable as new information is shared and discovered.
WHAT WILL YOU, AS MY THERAPIST, DO?
My work as a therapist is to facilitate and guide you towards making choices that help you grow and expand, rather than working to fix or change you. As such I will help you to understand what motivates you towards change, as well as what gets in the way of the change you desire. I will be your ally, supporting you with empathy and compassion. I will be your mirror, providing you an honest reflection of who you are now. I will be your coach, gently challenging you to move towards a happier, healthier, more confident YOU.
ARE YOUR SERVICES CONFIDENTIAL?
All services I offer are confidential. There are a few situations that legally require me to break confidentiality:
- If someone tells me they are going to harm themselves or someone else,
- If someone reports current child abuse, current elderly abuse, or
- If my records are subpoenaed by a court.
